If your NDIS plan includes Support Coordination, you can use this funding to work with a Support Coordinator to help implement your plan and connect with the right supports. The level of funding in your plan will determine the type and intensity of support you receive.
Which plan management type do you have?

NDIS managed 
 (Agency managed)

Mercy Connect is a registered NDIS provider, you can use us with agency-managed funding.

Plan managed

Send your plan manager our details and they’ll handle the invoicing.

Self managed

Self-managed participants can use any provider. We’ll invoice you directly.

Not sure what type of plan management you have? Not sure if you have this in your plan? Check under ‘Core Supports’ in your NDIS plan or myplace portal. If it’s not there but you think it should be, speak to your LAC or Support Coordinator at your next plan review.

Don’t have this in your plan yet?

If Support Coordination is not currently included in your NDIS plan, you can request it at your next plan review. Your Support Coordinator, LAC or support network can help explain how this support will assist you to understand and manage your plan.

If you are new to the NDIS, you can discuss your need for help navigating services and coordinating supports during your planning meeting so it can be considered in your plan.
